color: #000000;\">Other Official State or Federal Documentation<\/span><\/span><\/li>\n<\/ul>\n<div><span style=\"font-family: arial,helvetica,sans-serif; color: #000000;\"><span style=\"font-size: small;\">Important Note:<\/span><\/span><\/div>\n<div><span style=\"font-family: arial,helvetica,sans-serif; color: #000000;\"><span style=\"font-size: small;\">By signing the application, an applicant swears that the information given is correct;<\/span><\/span><\/div>\n<div><span style=\"font-family: arial,helvetica,sans-serif; color: #000000;\"><span style=\"font-size: small;\">Taking a false oath or making false statements to a public official is a Class D crime;<\/span><\/span><\/div>\n<div><span style=\"font-family: arial,helvetica,sans-serif;
