Greetings All,   I could use your assistance in reviewing the  attached 
draft survey.   Please consider what might be missing for  information that 
could be useful and also consider if there is stuff here we do  not need.  Any 
and all suggestions welcome.   If your comments  could be returned to me by 
Sat. Dec. 12 before the season gets any more  hectic,  that would be great.
