Request for Proposals for
Construction and Related Services Of Health Care Center
NOTE: The deadline for acceptance of this RFP has been extended to September 28, 2017.

The Town of Long Island is accepting proposals for the construction of a Community Health Center and related professional services associated with said construction including electrical and plumbing.
The winning proposal(s) will perform the required work as independent contractor(s) and be responsible for complying with any and all Federal and state laws.
This project is partially funded with a Department of Housing & Urban Development (HUD) Community Development Block Grant (CDBG). As such contractors must comply with all applicable Federal laws and regulations, especially the wage requirement rates as stipulated in the Davis Bacon Act.
Summary of these requirements is attached as Appendix A.
The selected contractor will construct in the basement of the Town of Long Island School a Community Health Center. Architectural drawings are attached to this RFP. Printed copies of these drawings are available at Town Hall.
The selected contractor will provide and maintain at all times insurance coverage amounts for Worker’s Compensation; a $1,000,000 comprehensive general liability policy; a $1,000,000 automobile liability policy and $1,000,000 in errors and omissions coverage.
The selected contractor is expected to provide and be solely responsible for the costs of all personnel, labor, machinery, equipment and fuel to provide the services and work.
Proposals should specifically list hourly rates for personnel as well as a total project price.
The Town reserves the right to waive or amend any requirement of this RFP and to reject any and/or all submitted proposals/bids. Work will be monitored by the Aging In Place Committee who may employ a Clerk of the Works for the duration of the construction.
